Job Description

  • We have an exciting new opportunity of an opening for a new home which means we are looking for Supported Living Enablers to support people to live as independently as possible in their home, applying the principles of Active Support to maximize autonomy and inclusion.

    You will be responsive to the individual needs and aspirations of the person receiving support, liaising with relatives, friends and professionals as is appropriate.

    You will adhere to high standards of care practice maintaining the provision and promotion of a safe, supporting and nurturing environment.

    You will support the aims and objectives of the organization, in line with the services policies and procedures.

    You will take a proactive stance in personal development working to achieve care qualifications, whilst furthering your knowledge and enhancing your skills, in line with organizational requirements and essential standards.

    Care Responsibilities:

    Your main responsibility is to assist in the delivery of a high standard of care to the people using Cream Care Supported Living Services; promoting each person’s dignity and their human and civil rights at all times.

    • At all times to work in line with the organization's Safeguarding of Vulnerable Adults policies and procedures, ensuring that any concerns are reported immediately.

    • Supporting people with their care needs in line with their care plan and commissioned care requirements. This may include aspects of their personal care; assisting with pressure areas, skin, teeth, mouth, hair, nails, dressing and undressing and supporting with personal hygiene requirements.

    SUPPORTED LIVING ENABLER

    • To treat people with respect and encourage them to express their individuality.

    • To be flexible and responsive at all times to meet people’s changing needs.

    • Assist people with their day to day lives, accessing meaningful opportunities and community participation where this is relevant to the person.

    • Assist people to eat and drink and to prepare meals whilst being aware of nutritional needs and cultural requirements. This is based on principles of Active Support to promote independence and an enabling rather than caring ‘for’ culture.

    • Keep accurate records of all care provided and ensure that this is managed with sensitivity and respect for people’s privacy and dignity.

    • Report immediately to the CCSL coordinator or the Service Manager any significant issues or changes to a person’s needs.

    • To act as a positive role model at all times

    • Support in the administration of medication in line with medication policy and the individuals care plan

    • Be prepared to undertake a keyworker role and responsibilities where this is of relevance to the person.

    • Develop positive relationships with family and carers where this is of relevance to the person.

    • Assist in identifying, supporting, and meeting the social and recreational needs of people in line with their care and person centered plans.

    • You will support in attending care reviews and liaising with relatives and professionals.

    • You will maintain confidentiality and follow the requirements of the Information Management policies at all times.

    • You will ensure you are familiar with each person’s care plans and risk assessments and support them in reviewing, updating and implementing changes to these where necessary.

    • Assist in the transition process for people new to CCSL services, recording and communicating information to allow for smooth stress free transition.

    • Supporting the person to maintain their home in the way that they wish, helping to undertake general housekeeping tasks with the person where needed

    • To carry out responsibilities under Health and Safety Act 1974.

    • Support the person to maintain their home and their tenancy, liaising with the CCSL Coordinator and Landlord where repairs or maintenance are required.

    • Adhere to all the organizational policies and procedures.

    • Enter actively in to supervision and appraisals processes to ensure your ongoing professional development.

    • To behave in a manner that reflects positively on the company at all times.

    • Adhere to the GSCC Codes of Practice and CCSL Codes of Conduct

    • Attend training courses to enhance your skills, including, all mandatory training and relevant Health and Social Care Diploma. Attend or participate in training specific to the needs of the person you are supporting as directed by the organization.

    • You will be required to work flexibly on a Rota basis to meet the needs of the service, this includes sleep-ins, weekends, and Bank holidays.

    • You may be required to work on a ‘lone – working’ basis and will be supported with a relevant risk assessment in order to do so.

    Please note that a Full, Clean, Drivers License is essential for this role.

Requirements

- To communicate, listen and relay correct information

- To be able to work as part of a team and also use own initiative when lone working

- Understanding and application of duties and responsibilities of a care role

- Experience of Electronic Care Recording Systems


Previous Experience:


- Experience within a care setting, preferably supporting those with a learning disability (Essential within a lone working environment)

- Experience in a Supported Living setting (Essential within a lone working environment)

- Person Centered Approaches

- Active Support Qualifications

- Basic numeracy

- Ability to communicate in written and spoken English

- Care related qualification, e.g. Diploma or Apprenticeship in Health and Social Care or commitment to undertaking one

- Care Certificate


Other Requirements:


- Able to work on a shift pattern, including early starts, late finishes, weekend and public holidays

- Valid applicable driving license
